The Nephrology Department at LoCicero Health provides expert, comprehensive care for patients with kidney disease through a highly collaborative, interdisciplinary model. Our focus is on early detection, effective management, and prevention of complications, particularly in patients with chronic kidney disease (CKD) resulting from diabetes and hypertension.
Understanding that CKD is often part of a complex health picture, our nephrologist works closely with the Endocrinology and Primary Care departments. This collaboration ensures that each patient receives coordinated care that addresses the root causes of kidney disease. Our endocrinologist helps manage diabetes, optimizing blood sugar control to reduce stress on the kidneys, while our primary care team coordinates long-term care, manages blood pressure, and addresses other cardiovascular risks.
Our interdisciplinary approach extends beyond physicians. Each patient’s care team includes a registered dietitian who specializes in renal nutrition. Diet plays a critical role in slowing the progression of CKD, especially in patients managing both diabetes and hypertension. Our Dietitian provides personalized nutrition plans and counseling on managing sodium, potassium, protein, and fluid intake.
A clinical pharmacist is also integral to our team, ensuring safe and effective medication use. They conduct thorough reviews to adjust dosages for impaired kidney function, minimize drug interactions, and support medication adherence. Our pharmacist also helps manage medications for hypertension, blood glucose, and anemia—common issues in patients with CKD.
| Early-Stage CKD
For patients with early-stage CKD, we emphasize proactive care and lifestyle modifications. Regular monitoring of kidney function, including glomerular filtration rate (GFR), blood pressure, and protein levels in the urine, helps detect worsening disease early. In high-risk populations, especially those with poorly controlled diabetes or longstanding hypertension, we provide more frequent evaluations in collaboration with other departments.
Education is a core part of our care model. We empower patients through individualized education on disease progression, treatment options, and lifestyle changes. This helps improve adherence to care plans and outcomes and delays the need for dialysis or transplant.
