Our Division of Primary Care is dedicated to providing comprehensive, patient-centered care. Board-certified internal medicine physicians and advanced registered nurse practitioners (ARNPs) evaluate, treat, and monitor patients’ overall health and wellness. Providers manage common diagnoses, order initial lab and diagnostic tests, and coordinate referrals to specialists when needed.

A key focus of our care is the management of chronic diseases such as diabetes and hypertension, which require ongoing attention and support. Our team has been successfully using a collaborative care model for chronic disease management. This includes physicians, ARNPs, registered nurses (RNs), a dietician, and a pharmacist — all working together to provide well-rounded, evidence-based care.

We are also continually enhancing our disease management efforts through targeted programs, such as our model Diabetes Clinic, designed to help patients stay up to date with necessary lab work and diagnostic testing.

We believe that strong collaboration between our primary care and specialist teams ensures greater continuity of care, better outcomes, and a more seamless experience for our patients.
